Two things decide whether epilepsy treatment works, and neither is the choice of drug. The first is never stopping suddenly: abrupt withdrawal of any antiepileptic can trigger seizures worse than the original ones, so a supply gap is a reason to call your doctor the same day, not to wait and see. The second is pregnancy planning. Sodium valproate causes birth defects in roughly one in ten exposed pregnancies and developmental problems in three to four in ten — the highest risk of any common antiepileptic — and carbamazepine both carries its own risk and stops hormonal contraception working. Any girl or woman of childbearing age on epilepsy medicine deserves that conversation before it becomes urgent, not after.
