Ciprofloxacin vs Ofloxacin (eye & ear drops): the fluoroquinolone twins — is there a real difference?

For years 'Cipro' was Nigeria's typhoid drug of choice, with ofloxacin (and its cousin levofloxacin) close behind — and that popularity is exactly why both now disappoint so often. Widespread, often incomplete use has bred fluoroquinolone-resistant typhoid and gut bacteria across West Africa, so a fever that 'didn't respond to Cipro' increasingly reflects resistance or a wrong diagnosis rather than a fake drug.

Between the two, differences are modest: ciprofloxacin has the edge on gram-negative gut organisms; ofloxacin achieves strong urinary concentrations and appears in many eye and ear drop brands. Both share the family's cautions — tendon injury risk (notably in older adults and with steroids), avoidance in pregnancy and children except when a specialist decides otherwise, and interference from antacids, milk, and mineral supplements, which can block absorption almost completely if taken together.
